provided by physicians and supplementary health care practitioners,

Ontario OHIP
A resident of Ontario must have a health card to show that he or she is
entitled to health care services paid for by OHIP. The Ministry of Health
and Long-Term Care pays for a wide range of services; however, it does not
pay for services that are not medically necessary, such as cosmetic
surgery.
Alberta Health Care Insurance Plan
The Alberta Health Care Insurance Plan (AHCIP) is a publicly administered
and funded health care system that guarantees Albertans receive universal
access to medically necessary hospital and medical services. AHCIP forms,
information on premium and drug assistance, answers to common questions
and more are available.

Passport Canada
As mandated by the Canadian Passport Order, Passport Canada is responsible
for issuing, revoking, withholding, recovering, and providing instructions
on the use of Canadian passports. It provides guidance to missions issuing
passports abroad and supervises all matters relating to Canadian travel
documents.
Passport Canada has four regional
operations: Western, Ontario, Central and Eastern. There are 33 passport
issuing offices across the country from St. John's, Newfoundland and
Labrador, to Victoria, British Columbia.
